Active Surveillance report
Laboratory-based surveillance
SLAB [program ID] ([species] [number of animals]...) [location] {lab ID}
- This Lab-based active surveillance report always starts with SLAB.
- Surveillance program ID is created by iSIKHNAS during the planning phase of the activity via the iSIKHNAS website. This will be provided to you by your coordinator.
- Use KODE SP to find the species code from the species code list.
- Number of animals of this species sampled.
- Sequence may be repeated for multiple species.
- Use CKL [name of location] to find the location code where activity takes place.
- Optional: Lab ID code where the specimens will be sent. Use CKI LAB [location code] to find the Lab ID codes in your area.
Field-based observation surveillance
NOTE: This tool is extremely useful in certain circumstances where clinical signs are easily observed and need to be reported (for example, in a priority disease investigation) but in reality this tool will be rarely used.
SLAP [program ID] ([species] [total positive] [total negative]...) [location]
- This field based surveillance report always starts with SLAP
- Surveillance program ID is created by iSIKHNAS during the planning phase of the activity via the iSIKHNAS website. This will be provided to you by your coordinator.
- Species code. Use KODE SP to find the species code from the species code list.
- Total number of animals with the observable sign
- Total number of animals without the observable sign
- Sequence may be repeated for multiple species.
- Location code to the desa level where these observations were made. Use CKL [name of location] to find the location code where activity takes place.
