Formulir PDSR 1 & 2 and Formulir Rabies 1 & 2 are forms adapted from FAO PDSR Program forms in an exercise to see how well iSIKHNAS could be used as a data collection tool in conjunction with PDSR forms. These are still very much under discussion.
